Rehabilitation does not end at discharge. The hardest stretch begins exactly when the person returns to the same city, the same job and the same people — only now without the substance.
In the centre a person lives in an environment where everything supports sobriety. At home that environment is gone: the old acquaintances are still there, the old routes through town, the old role in the family, and the same occasions that started it all.
There is one more thing people are rarely warned about: disappointment. They expect life to fall back into place the moment rehabilitation ends — the family will forgive them, work will appear, the money will come back. In reality trust takes years to rebuild, and it is in that gap between expectation and reality that most relapses happen.
A supported return to an ordinary rhythm: a daily routine, household duties, work, responsibility for their own decisions. The aim is for the person to learn to cope with ordinary life, not only with life inside the centre.
Then comes a sobriety plan for each day: what to do with a free evening, who to call the moment a craving starts, which places and which company are ruled out completely. It sounds mundane, but it is the mundane things that hold the result in place.
Separately, there are recovery groups and fellowships. Someone left alone with their sobriety holds on to it far less well than someone who regularly sees other people walking the same road.
