How drug addiction is treated: where to start and how substances differ

Treatment does not start with a drip; it starts with a conversation. The hardest stage is persuading someone who denies to the last that there is any problem at all. Here is how that is done, and what depends on the type of substance.

The first step: the family speaking with one voice

A person who is using almost always denies both the use itself and any dependence on it. This is not stubbornness, and it is not lying in the usual sense: denial is a mechanism of the illness, and the person sincerely believes it. That is why appeals from one relative at a time rarely work: one insists, another feels sorry for them, a third hands over money, and the whole thing falls apart.

What does work is different. The family agrees on a single position beforehand and then insists together on residential treatment. It is worth preparing for that conversation — knowing already exactly where the person will go, and telling them the main thing: treatment is anonymous and there will be no judgement.

If the family cannot reach agreement, a motivational psychologist is brought in and visits the home. This is ordinary practice, not a last resort: a specialist speaks to the person differently from the family, because they share no history of old grievances.

Why the type of substance makes a difference

Substances differ in how they act on the nervous system, and that shapes the course of treatment. Broadly, they fall into three groups.

Some substances are plant-derived, some are made in a laboratory, and most of what circulates today is a mixture of the two. For a family that means one practical thing: nobody knows what is in a street substance, including the person taking it.

Treatment is hardest for people using opioids: heroin, morphine, desomorphine and the like. But what decides the outcome is still not the substance; it is the person’s own willingness. Any method works far better when someone wants to stop.

Physical and psychological dependence are two different things

Physical dependence is the way the body reorganises itself after prolonged use. It shows itself as withdrawal: a severe physical and mental state when use stops. Relieving it with another dose only starts the cycle over, so this stage is done under supervision.

Psychological dependence is the pull to return to pleasant sensations and to blunt anxiety and problems with a substance. It does not disappear after detox, and it is the reason a person relapses when they are already physically well. It is treated not with medication but with work on thinking and behaviour.

Can it be treated at home?

At home it is sometimes possible to deal with the physical state, and even that does not always work. But the same people, the same places and the same triggers are still there, so a return to using after a home detox is a common story. Residential treatment exists for the second, much longer part of the work.

The person flatly refuses. What can we do?

Do not try to persuade them one relative at a time. First agree as a whole family, prepare a specific centre to offer, and if the conversation is going nowhere, bring in a motivational psychologist who will come to the home.
